Welcome to the Heritage Health Office! My goal at Heritage is to keep my students healthy, happy and in class. Heritage Middle School health services will meet the New Jersey State standards. All students will be screened for vision, blood pressure, height and weight (BMI). Students in grade 7 will be screened for auditory acuity and scoliosis. Health education and counseling, medical referrals and follow-up care are provided as needed. Emergency Care Plans are written for each student with the potential for Anaphylaxis. Individual Health Care Plans are also written for students with chronic health conditions. I hope to provide maximum health services to students, faculty and the community. Please inform the nurse’s office of any changes in your child’s health. ![]() Fall Sport Physicals will be offered at LHS on Wed., June 12, 2013 @ 4PM Fall Sport Forms are due August 1, 2013 THE HEALTH HISTORY, EMERGENCY FORM, PERMISSION FORM, SPORTSMANSHIP PLEDGE AND CONCUSSION FORM MUST BE COMPLETED EACH SEASON BEFORE TRY-OUTS FOR ALL SPORTS. Spring Sport Physicals will be available at LHS on Wednesday, January 30, 2013 at 4PM for students who are unable to get a physical examation by their pediatrician. Physical Examinations can NEVER be more than 1 year old while participating in a sport. Deadlines are set up which assure for timely and accurate approval of students for each sports season. Students who submit accurate and complete medical forms by the deadline date have the assurance that their approval will be completed by the first try-out! The following forms must be completed and given to the nurse PRIOR to try-outs for each sport at Heritage Middle School as per N.J.A.C. 6A:16:2.2: An ASTHMA ACTION PLAN must be completed for students with asthma! Our School Physician reviews each HEALTH HISTORY AND PHYSICAL EXAMINATION FORM before a student is permitted to participate in any school sport as per N.J.A.C.6A. Sport physical forms must be signed, stamped and dated. Incomplete forms will not be accepted and must be returned to the attending physician. This includes height, weight, blood pressure, vision and pulse. Each sports season, we need to assure that your child's health has remained the same. If your child's health has changed or he/she has been excused from PE for more than one week, the physician will need to re-verify approval for sports. This will keep your child safe and healthy during sports participation at school. Sport physicals will be offered on the following dates at LHS.
