Sports Physicals

Students wishing to participate in interscholastic or intramural sports must submit the appropriate forms to the Health Office prior to the sport season.  All forms are available for download and include:
 
 
  • Annual Athletic Pre-Participation Physical Examination Form (Required by the State of New Jersey for students in grades 6-12 participating on a school-sponsored interscholastic or intramural athletic team or squad and must be completed prior to a student’s participation as per N.J.A.C. 6A:16 Programs to Support Student Development.)
  • Concussion Policy (click here for more information)
  • Athletic Emergency Card
  • Sportsmanship Pledge/Permission Card/Steroid Testing (random testing will only take place with high school students)
IMPORTANT NOTE:  If a student has asthma, an Asthma Action Plan must be on file in the student's health records.  If you have not already done so, please have an Asthma Action Plan form completed and return it along with all other paperwork.

Included in the State of New Jersey mandate regarding participation in sports, physicals are valid for one calendar year only.  All physicals MUST be documented on the official State of New Jersey physical form which is available for download (see above) or in the health offices at Mt. Pleasant Middle School, Heritage Middle School and Livingston High School. 
 
Sports physicals may be completed by a student’s personal physician or if you cannot afford to have a physical examination done or do not have a family physician, school physicals are offered prior to each season.   All other forms must be completed and brought with the student at the time of the school physical.

These physicals are performed in the Health Office at Livingston High School.  
 
 
If your personal physician is administering the physical examination, his/her findings
must be recorded on the official Athletic Pre-Participation Physical Examination form.
 Please be sure to complete all appropriate sections and have a parent/guardian sign the form prior to visiting your physician.
 
The due date for submission of all LPS forms to the school health office
is the same date that the season's physicals are given
.
 
NOTE:  Students are strongly encouraged to submit physicals as soon as possible prior to the beginning of the season.  If you are unable to adhere to the deadlines stated above, there is a strong possibility that you will not be cleared in time due to the high number of physicals that need to be processed.
