    HMS Athletic Announcements:
     
    Boys Lacrosse - Congratulations in placing second place in the Tournament.  
    Please return your uniform by Friday, May 23rd to Mrs. Mullin. 
    Baseball - Congratulations on advancing to the semi-finals.   Semi-final game is tomorrow, Wednesday, May 21st @ Frelinghusyen Middle School.

    Boys Tennis - Practice today, May 20th and tomorrow, May 21st from 3:15 to 4:45.  Please return uniforms to Mrs. Marzano by Friday, May 23rd.
    Track - Practices on Tuesday, May 20th and Thursday, May 22nd from 3:15 to 4:45 and Wednesday, May 21st from 3:15 to 4:30.  (weather permitting)

     Effective July 1,2024, the New Jersey Department of Education has mandated changes for eligibility to participate in athletics during the 2024-2025 school year. Please read all the instructions below carefully:

     

    Step 1: Registration  

    Registration is completed via the Genesis Parent Portal under the forms tab and look for the applicable season and year’s Heritage Middle School Athletics Sign-up 

    (i.e. “Spring 2025 Heritage Middle School/YMCA Athletics Sign Up”).


    Step 2: Medical Requirements

    Students are required to complete the Preparticipation Physical Evaluation Form ONE TIME per calendar year. It is valid for 365 days.  This form must be completed and only the second page of this packet, the medical eligibility form, is returned to Heritage Middle School nurse. Please keep a copy for your records. Please do not email forms.


    For every new athletics season during the school year (Fall, Winter and Spring), the student's physician is required to complete and retain the Athletic Health History Form .  Only the third page of this packet, the medical eligibility form, will be returned to Heritage Middle School nurse. The first two pages remain on file at the physician’s office.


    The Heritage Middle School sports program is funded by the West Essex YMCA and there are participation fees involved for each season.  When tryouts have been completed, there will be a link sent with instructions on how to submit payment directly online to the West Essex YMCA.

     Please note:  In order to participate in tryouts and practices, medical forms need to be submitted to the school nurse and forms need to be completed on the Parent Genesis Portal.
