Procedure for Submitting Working Papers During the Pandemic
To submit working papers via email, please follow the instructions below:
1. Students should complete all of Section A2. Employer will complete Section B3. Doctor will complete Section C
Students should sign the bottom right hand side of the form (signature of minor).Students should fill in the last grade completed on the bottom left of the form.
Once that is all complete, scan the form and email it to firstname.lastname@example.org, along with a copy of the student's ID (birth certificate or passport).
The completed form will be scanned back to you. Students should bring the completed form to his/her employer.
If the job is out of the state of New Jersey, the student must obtain working papers from that state and follow those instructions.
