PRESCHOOL REMOTE REGISTRATION INSTRUCTIONS:

• Only a parent/legal guardian domiciled in Livingston may register a new student for Livingston Public Schools. View proof of residency requirements. 

Before you start the PreSchool Pre-Registration online, please note the following:

• To complete and finalize your child’s pre-registration, all information and documents must be uploaded to your pre-registration account. The Registrar will contact you ONLY if she has any questions regarding your information or documents.
• The following documents are required to create a pre-registration user account (not to be confused with an existing Genesis Parent Access portal account): 
  1. Original Birth Certificate
  2. Print out of Current Immunization Record. Please contact the Burnet Hill School Nurse for requirements regarding the influenza vaccine at 973-535-8000, ext. 6925.
  3. Parent completed Confidential Medical Forms
     • Confidential Medical Form
     • Mantoux Form
  4. Three (3) proofs of residency
  5. Photo Identification for Parent/Guardian 1
• In order to complete the registration process online make sure that you receive an email confirmation BEFORE LOGGING OUT. For PreSchool Referral families requesting an evaluation, your child's case manager will contact you to schedule an identification meeting. For the Integrated PreSchool Program, you will hear from the Student Services Department with next steps.  

PLEASE NOTE THE FULL LIST OF REQUIRED DOCUMENTS BELOW

• Original Birth Certificate
• Health Services Information/Doctor’s Medical Packet  (within a year of starting school)
• Parent Completed Confidential Medical Form and Mantoux Form
• Proof of Residency (3 proofs of residency are required)
• Photo Identification for Parent/Guardian
• Landlord Affidavit (if applicable)
• If Parents/Guardians live at more than one residence, regardless of which parent has legal custody, court documentation of the custody agreement must be supplied. If a legal custody agreement does not exist, Forms 218A and 218B must be completed.
