Absence Request for Extraordinary Opportunity
On August 11, 2008, the Livingston Board of Education approved Policy #5200 which deals with pupil attendance. For a full reading of this policy and regulation, click here.
NOTE: Family vacations should be planned during school vacation periods. The school calendar is published at least one year in advance for this purpose. However, should an extraordinary opportunity arise there is a process to request school approval. Parents are requested to submit a "Request for Extraordinary Opportunity" form to the school principal.

Asthma Treatment Plan
If a student has asthma, an Asthma Treatment Plan must be on file in the student's health records. Please return the completed to the school nurse.

Confidential Medical Information Update Form
For returning students, a Confidential Medical Information Update Form needs to be completed and signed each school year. This form (one per child) must be completed, signed and returned to school by the end of the second week of school.

Free and Reduced Lunch Program
The State of New Jersey requires that school districts provide a subsidized lunch form to all parents/guardians. Download this packet which includes information on eligibiity and instructions on how to apply. If you feel you may be eligible, please complete the application form and send it to the Business Administrator's Office, 11 Foxcroft Drive, Livingston, NJ 07039, by the end of the second week of school.

Health History Update Questionnaire
To participate on a school-sponsored interscholastic or intramural athletic team or squad, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide a health history update questionnaire completed and signed by the student’s parent/guardian.

Integrated Preschool Program Application
Livingston Public Schools has two pre-school programs available to three- and four-year olds. One program offers a self-contained setting for special education students known as PRIDE and the other offers an integrated setting with special and general education students.

Release of Records for Students Transferring to Private School
If you plan on withdrawing your student from the Livingston Public Schools to attend a private school, please download and complete this form. After completion, please give it to your child's school office.
